Trust me this is how it works in the real world with real companys, if said person has made a change without authority then appropriate action should be taken. This would be done behind closed doors

Disagree completely as it happens - The MD (Equiv position) of my company and the Chairman don't sign off technical and detailed level changes like that, you have operational management for that and at that level those guys have to be accountable for detailed level changes.
Short sighted view - that's like saying it is the pilot's fault or Richard Branson's fault that a Stewardess called me a pillock and spilled a drink on me on a Virgin flight? Neither of those people, though both senior and operationally in charge of the experience and person in question are their line managers appoint or vet them.
The only thing I agree with is that if someone has ventured off piste, then it is an internal disciplinary matter.
